Security ID Badges

Badges made at Security Service Center, NBE47
Monday-Friday, 7:00 a.m. -8:00 p.m.

To apply for a badge, complete the appropriate Security ID Application below. A picture ID is required.

Download the form (requires UMMC INTRANET access), fill it out, and bring it to the Security Service Center, NBE47.

On-line Alternate

You may fill out this form on-line, then click "Save As" to save it to your computer, and print a copy to bring to the Security Service Center, NBE47.

Hospital Rules

  • Security ID Badges are to be worn conspicuously displayed at all times while in the Medical System Buildings. Wear the badge on the upper part of your body where others can easily see it.
  • Unauthorized use of Medical Center ID Security Badges may result in Corrective Action or revocation of the badge.
  • The Security ID Badge may not be loaned, transferred, or used by anybody other than the person who is pictured and named on the badge.
  • You are responsible for replacing your badge if it is missing or damaged.
  • The replacement cost for a badge is $15, effective July 1, 2009.
  • A new badge may be obtained without charge after two years if all prior badges have been returned or resolved.
  • A new picture will be required for all new badges issued if the existing picture is more than three (3) years old.
  • Security ID Badges are the property of the University of Maryland Medical Center and must be surrendered to the Medical Center upon termination, end of contract, or when requested by Medical Center management.

PowerChart/FirstNet

Residents and Fellows will complete a 4 hour online training module. No Instructor-Led Training is offered.

Email IS&TTrainingteam@umm.edu to reserve one of the following dates and times for your residents/fellows who require this training:

  • June 9, 10, 12, 13, 16, 18 (June 18th from 8a-12p only) and 20
  • June 24 (June 24th from 8a-12p only) and 27
  • June 30
  • July 7, 9, 10 and 11
  • July 14, 16

The modules are available offsite however, the training may be completed in the Paca/Pratt Building (110 S. Paca St., 5th floor, Baltimore, MD 21201).

Sessions times:

Morning Session: 8:00 AM – 12:00 PM
Afternoon Session: 1:00 PM – 5:00 PM

The Hampden Training Room (seats up to 32) is reserved for both Morning and Afternoon sessions to be held each day unless otherwise noted:

How to obtain a PowerChart Username and Password:

  • A certificate of completion will be emailed to the Resident/Fellow after passing the exam.
  • Resident/Fellow needs to show it to the coordinator as a proof of training completion.
  • Coordinator and Resident/Fellow prepare the Account Request form.
  • Fax the form to 410-328-5499
  • Call the Helpdesk at 410-328-HELP (4357), if the Resident/Fellow has  not received the PowerChart account in 3 days.

Kernan Inpatient Training

  • One 4 hour session

Registration Process:

  •  By email:
  • Please provide the following information:
    • Your name and department
    • Dates and class times you are requesting
    • Number of residents and a list of attendee names and their role (resident or fellow)
  •  Kernan inpatient: Request a class for the specialty using the registration process.
  •  Request training by logging onto Portfolio
    •  Complete a class registration form. A trainer will respond to your request
