After Your Visit

After your visit, you may need to obtain your medical records information for care in your hometown.

Procedure for Requesting your Medical Records:

  • Complete the authorization for release of information, please visit our How To Receive Copies Of Your Medical Records page to receive and print a copy of this form
  • Submit the authorization for release of information form by mail or deliver in person to:

University of Maryland Medical Center
Department of Health Information Management
22 S. Greene Street
Baltimore, Maryland 21201

Form can also be faxed to 410-328-0537 between the hours of 8:00am - 4:30pm, Monday - Friday

  • To obtain records of a deceased patient, please obtain and submit the Letter of Administration which can only be received by the patient’s next of kin from the probate court in your state
  • Receive invoice and mail or deliver your check or money order payment to:

Cash payments will only be accepted at M&T Bank located on the 1st floor of the Medical Center between the hours of 8:30am – 4:00pm

  • Receive records within 10 days of receipt of payment

Resources For Your Hometown Doctor:

After your visit, your doctor may need to obtain your medical records information for care in your hometown. For questions that your doctor has regarding your medical record, the doctor or their office can contact:
